What Youll Do Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program. Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies. ‍️ Early Intervention: Monitor symptoms, identify risks, and escalate care before complications arise. Coordinate Care: Work seamlessly with primary care providers, specialists, and community resources. Patient Education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.
