Become a part of our caring community Humana's Primary Care Organization is one of the largest and fastest-growing senior-focused, value-based care providers in the country. With over 400 centers across 15 states operating under the CenterWell and Conviva brands, we're transforming healthcare by putting seniors at the heart of everything we do. As a Physician Collaborator you will share our vision to improve health care in the communities we serve! This is a fully remote role, with an average of 5 hours of work monthly. You will report to our Associate Director. You will collaborate with our APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months when they cannot get in with their own PCP. How you will contribute: You will collaborate with the APP to close gaps in care and ensure access to needed services You will complete regular chart reviews and coordinate monthly meetings You will be a resource for APPs to ensure quality patient care Use your skills to make an impact Required Qualifications Must have unrestricted medical license in the following states: KY, TN, MO, NC, TX, IN Graduate of MD/DO program of accredited universit…
